The Medication Therapy Management (MTM) Program is a service for individuals taking various drugs and suffering from multiple health issues. The program ensures that you are taking the proper meds and that they are improving your health.

What do you obtain?
Those who qualify for the MTM Program will receive the following:
- A letter of introduction detailing how to get started.
- Comprehensive medication review:
- Each year, you will have the opportunity to discuss your medications with a highly-trained pharmacist. Or a pharmacist intern working directly under the supervision of a pharmacist. This analysis will take between 20 and 30 minutes. We will schedule the call during a time that is convenient for you. You can discuss any difficulties with your medications during this conversation.
- After completing the comprehensive medication evaluation, you will receive a summary by mail. The summary has a medication action plan with space for you to make notes or ask any more questions.
- In addition, you will receive a Medication Review letter that contains a summary of all the medications you take and the reasons for taking them.
Pharmacists give Medication therapy management (MTM) service to eligible Medicare part D beneficiaries who enroll in a Medicare prescription drug plan. In the event of a significant disparity or if the pharmacist believes their patient requires consultation services, core element four, intervention and referral, can implement.
